Abstract
Background and aims: Complex regional pain syndrome (CRPS) remains poorly understood. Some patients develop a chronic disabling condition. Recent studies report poor outcomes at 1 year: 51-89% still present some features of CRPS, 14 to 27% meet Budapest criteria, while only 60-70% has returned to work. Some risk factors have been associated with its incidence of CRPS but little is known about its (early) prognostic factors and how they influence long-term outcomes. The aim of the present study is to examine the role of biopsychosocial factors in the development of persistent CRPS. Methods: Early CRPS patients (< 6 months) are included in a prospective longitudinal observational study and assessed at 4 time points during 1 year (as soon as possible, 4.5, 6, and 12 months). Each session includes anamnesis, clinical examination, quantitative sensory testing, visuospatial attentional abilities and questionnaires covering the biopsychosocial model. The primary endpoint is the disability. Results: Recruitment is complete (113 participants); we plan to finish follow-up in December 2024. We would be in a position to give full results for the EFIC 2024 congress. Based on the current litterature and the inclusion characteristics of the cohort, we predict that body perception disorders, allodynia, sensitivity to pressure, and some psychosocial parameters will be the most important early prognostic factors. Conclusions: The final results will allow to better understand the evolution of early CRPS, and to investigate which of these biopsychosocial factors predict long-term outcomes in CRPS. Ultimately, it will allow patients to be stratified according to their risk of chronification.
