Abstract
Pain is a subjective experience and is, therefore, most probably conscious. Patients with a disorder of consciousness (in vegetative state or in minimally conscious state) constitute an interesting pathological model to investigate the relationships between the experience of pain and consciousness. In this view, a method of interest is the electroencephalographic (EEG) recording of brain responses elicited by stimuli conveyed either by the spinothalamic tract or the lemniscal tract. Specifically, characterizing brain responses evoked by transient cold stimulation of the skin combines the advantage that these responses are conveyed by the spinothalamic system even though they do not generate a painful sensation when delivered to healthy subjects. Technically, to generate those stimuli, it is required to use a thermode that is able to generate very steep cooling ramps when applied to the skin. Such a device has been made recently available and is usable at the patients’ bedside. The results from these analyses could have an important clinical and ethical impact in the care of severely brain-injured patients that are unable to communicate their perception, including pain
