Abstract
(en) Pharmacogenetics as a tool to improve therapeutic drug monitoring: experience at Cliniques universitaires Saint-Luc with administering immunosuppressive drugs to renal transplantation patients] Despite being highly effective, certain drugs exhibit considerable side-effects, thus being characterized by a relatively narrow therapeutic index. For this specific reason, it is at times required to regularly check these drugs’ blood concentration in order to find the finest compromise between optimal therapeutic efficacy and reduced toxicity. Therapeutic drug monitoring (tDM) is applied to immunosuppressants used in solid organ transplantation. Compared to conventional tDM, a first improvement consists in understanding the origin of interindividual variability at the pharmacokinetic level. This would allow for better anticipating this variability by proposing individualized dosages according to each patient's genetic characteristics. Another improvement, complementary to the first, consists in directly measuring the drug’s active biological dose in target tissues, such as lymphocytes in our case, while simultaneously studying genetic and other factors impacting this parameter. As a complement to conventional tDM, pharmacogenetics is instrumental in enabling a better individualization of drug therapy.
