Abstract
Post-stroke spasticity impairs walking function and quality of life. This thesis aimed to improve its assessment and to study the effects of repeated cycles of higher doses of botulinum toxin on gait function. An instrumental method for quantifying muscle stiffness was validated in post-stroke spastic individuals, showing good reliability. In addition, the effects of three cycles of progressively increasing doses of botulinum toxin were studied in individuals with multifocal post-stroke spasticity, showing clinical and kinematic improvements in gait after repeated injections. These results highlight the relevance of repeated injections with sufficiently high doses of botulinum toxin to improve gait. An objective assessment of stiffness helps to distinguish the different components of hyper-resistance, the main clinical manifestation of spasticity. This work will help guide more targeted therapeutic strategies to support gait recovery and participation after stroke.
