Abstract
Introduction: Hemineglect can be defined by a spatial attention impairment that is augmented by distractor inhibition demands. However, the relationship between spatial and distractor inhibition attention remains poorly understood. We created the REAsmash immersive virtual reality serious game based on feature integration theory and implemented it on a device with integrated eye-tracking (ET). We studied for the first time how inhibition demands influenced visual spatial exploration contrasting results from post-stroke participants with and without visual hemineglect, relative to healthy controls. Methods: The REAsmash consists of a peripersonal search task with a cartoon-like 3D garden containing molehills from which a target mole and several distractor moles emerged. There were six levels containing different numbers (11, 17 and 23) and types of distractors (low & high target-distractor saliency). We tested three groups of participants (post-stroke hemineglect; post-stroke without hemineglect; matched controls). The independent variables were group, target-distractor saliency, distractor number and target laterality. The dependent variables were action response time and omissions, and ET metrics were used to measure visual exploration. Results: We found group effects showing longer reaction and cumulative fixation times in post-stroke groups relative to controls, particularly in low saliency levels with increased distractor numbers. Post-stroke groups also showed higher rates of capture and recapture fixations. Additionally, hemineglect individuals showed the same effects with lateral bias. Discussion: ET results help explain the longer response time and omissions in the post-stroke groups. The ET metrics show perseveration and working memory issues, with additional laterality differences for individuals with hemineglect.
