Abstract
(en) Objectives: Training health care professionals in therapeutic patient education is on of the most important means of developing educational practice in the long term follow-up of people living with chronic disease. However, such training cannot be limited to learning abilities in communication and educational techniques and skills. They also have to contribute to redefining health care professionals' roles, and moreover in changing their professional identity, from the dominant acute model of medicine and care, to chronic long term care. Methods: A training programme have been developed and applied to several groups of health care professionals. It aimed at improving communication and teaching skills as well as patient relationship and psychosocial abilities. Results: The post training evaluation shows that several changes in abilities, representations and practice have occurred. But it also shows that some abilities are uneasy to change, and would need more in depth work, such as abilities to explore, understand and integrate patients' representations of disease, treatment and self roles, abilities to negotiate health recommendations, treatments in order to make them more acceptable and feasible to patients, and abilities to let the patients become more in control (responsability and freedom of choice) of their life with chronic illness, including in making mistakes and self evaluation of their health.
