Recent developments of computer assisted surgery in maxillofacial surgery and especially in orthognathic surgery deal with the use of the third dimension to accurately plan and perform the surgery. However, all existing computer-assisted plannings in orthognathic surgery are executed without any three-dimensional (3D) reference system allowing for truly 3D diagnosis and planning. The goal of this PhD thesis was the de novo development of 3D computed tomography (3D-CT) based craniofacial cephalometric analysis. We searched for constructing a 3D-CT cephalometric analysis that could allow for providing a 3D reference frame for an accurate 3D diagnosis and computer-assisted 3D orthognathic surgery planning. The context of the research, concept, software development, experimental validations and possible applications of 3D-CT cephalometric analysis are described in six chapters. Chapter 1, "Introduction", provides more specifically the context of the research. Theoretical and practical problems arising during current orthognathic surgical planning are first presented. The development and application of a methodology common to clinicians and engineers allowed for discovery of non coherent transfer of 3D information (clinical examination, plaster casts) and the 3D model surgery through 2D diagnosis tool (2D cephalometry). Chapter 2, "Software toolkit for three-dimensional CT based cephalometric analysis," describes main computer tools developed to achieve a truly 3D cephalometric analysis based on 3D CT imaging modality. Chapter 3, "3D CT based cephalometric analysis: theoretical concept," describes the development of specific theoretical approach based on already existing 2D cephalometric background. Structure and use of 3D CT cephalometric analysis are also presented. Chapter 4, "3D CT cephalometric craniofacial analysis: Experimental validation in vitro" provides results of in vitro validation of 3D CT cephalometric method versus 2D lateral X- rays classical cephalometry. Chapter 5, "Accuracy of the three-dimensional craniofacial cephalometric landmarks on the low dose CT" presents a validation of a low-dose CT protocol which was especially created for radioprotection reasons for patients undergoing a 3D CT exam for 3D CT cephalometry purpose. Chapter 6, "Discussion and perspectives" presents the discussion in relation to problems such as 3D approach in sciences, conceptual bases of and information obtained from 3D cephalometry, development of a new craniofacial sagittal plane, and radioprotection issues. Potential applications of 3D CT based cephalometric analysis are also described.
