Abstract
(en) [Imported diseases: focus on fever in travelers returning from tropical countries] It is assumed that during travel in tropical countries, 70% of subjects have to face health problems, 8 to 15% require a consultation, and 5 to 10% present with fever. Of the latter, 27% will need to be hospitalized, 2% will be staying in intensive care units, and 0.5% will actually die. The patient’s anamnesis and thorough clinical examination are essential for establishing an accurate diagnosis in patients presenting with ever upon return from tropical countries. The primary tropical diseases responsible for fever include malaria, rickettsial infections, schistosomiasis with Katayama fever, dengue fever, enteric fever, and protozoan diseases. Under these circumstances, it is of key importance to rapidly exclude all conditions that may lead to patients’death, such as severe malaria by Plasmodium falciparum or highly contagious diseases. Any fever in travelers returning from tropical countries is caused by malaria, until proven otherwise. In an epidemic context, notably in travelers coming back from Guinea, Sierra-Leone, or Liberia, the possibility of Ebola virus infection must also be considered.
