- Ovarian tissue cryopreservation and transplantation is the only feasible option for prepubertal cancer patients to preserve their fertility before gonadotoxic treatment. Although the technique is promising, with more than 130 livebirths to date, a number of issues related to the transplantation procedure exist. First of all, there is a theoretical risk of reimplanting malignant cells within the ovarian tissue, leading to cancer recurrence. A second issue concerns the grafting procedure, which incurs to massive loss of quiescent follicles, a phenomenon known as follicle burnout. Finally, ovarian tissue from prepubertal patients has only been studied marginally, chiefly due to its scarcity. In this context, the aim of the present work was first to evaluate the safety of the transplantation procedure in patients affected by borderline ovarian tumors. The molecular mechanisms involved in massive follicle growth in the early days post-grafting have also been addressed and last, specific issues in pediatric ovarian tissue have been evaluated, with emphasis on mitochondrial function and content.
