Abstract
(en) [Sudden death in athletes: Who is at risk? How can at-risk patients be identified?] Though uncommon, sudden cardiac death in the athlete may occur at all ages. While numerous potential causes have been identified, the cardiac origin remains the main cause and should thus be investigated further. First, the cause of death appears to be related to the victim’s age (<35 years or >35 years). In the youngest patient group (under age 35), congenital heart disease, and especially hypertrophic cardiomyopathy, is the most common condition to account for the death. In elderly sportsmen, coronary artery disease is the most frequent death cause. Some other conditions related to the manner in which sports is undertaken should be considered, namely intensity of sports, competition, and potential doping, in addition to the atmospheric conditions. The arrhythmias responsible for the death are either ventricular fibrillation or rapid ventricular tachycardia in the majority of cases. Emergency cardiopulmonary resuscitation is required at once to reanimate the patient and improve survival. Given that the death is sudden, unpredictable, and often fatal, a suitable screening program able to detect individuals at risk of developing this tragic event is warranted. Yet the choice of clinical tests best suited to predict and prevent sudden cardiac death is still a matter of fierce debate.
