Transthyretin amyloid cardiomyopathy (ATTR-CM) is a severe and progressive restrictive infiltrative cardiomyopathy caused by the deposition of misfolded amyloid fibrils in the myocardium. Cardiac SPECT or SPECT/CT using 99mTc-radiolabeled bone-seeking radiopharmaceuticals complements echocardiography and cardiac magnetic resonance and has revolutionized the management of patients with suspected ATTR-CM. In particular, cardiac SPECT with bone-seeking radiopharmaceuticals allows to obtain a non-biopsy diagnosis of ATTR-CM with high accuracy if a plasma cell dyscrasia is excluded according to current international guidelines.
