- Abstract
- Notwithstanding the relatively low incidence of infective endocarditis, this pathology still remains a life-threatening disease that causes also significant morbidity. Preoperative analysis of the valve is a crucial requirement in order to choose the best surgical technique. After identifying the degree of valve involvement and after meticulous and complete debridement, several surgical techniques can be considered. In case of limited cusp destruction like small perforations, repair techniques could offer good results. Bicuspid valves deserve special mention as their particular anatomy requires ad hoc techniques in itself. When we are confronted with extensive cusp destruction without annulus involvement, valve replacement seems to have better outcomes. In the case of annulus abscess, surgery can get really tricky by requiring more complex procedures such as the use of composite grafts or homografts.
