Therapeutic advances over the past decades have enabled a growing number of patients with congenital heart disease (CHD) to survive into adulthood. However, pregnancy in these women remains a major challenge due to the increased risk of obstetric and neonatal complications. The underlying mechanisms of these complications are poorly understood, limiting our ability to predict and prevent them. This research aims to better understand these mechanisms through a combination of retrospective and prospective approaches. Our findings identify placental dysfunction as a central mechanism, revealing a higher prevalence of maternal vascular malperfusion and increased markers of placental hypoxia in women with CHD. These discoveries provide promising perspectives for improving the prediction and management of these high-risk pregnancies.
