Abstract
The molecular definition of tumor antigens recognized by T lymphocytes has opened the way to various immunotherapy approaches aimed at promoting immune rejection of the tumors. A number of clinical trials, mostly based on vaccination, have been performed in melanoma patients, and some of them yielded positive results, including the induction of complete clinical responses in metastatic patients. However, this was observed only in a minority of patients, most of them showing tumor progression. Some of the possible reasons for these contrasting results will be discussed as a prelude to suggesting ways to improve the rate of success in future trials. Topics discussed will include the choice of the antigen(s), the choice of the vaccination strategy, the understanding of the intracellular processing of the antigens, and the mechanisms of tumoral immune resistance.
