- Objective: to evaluate the ovarian response to stimulation conducted for IVF treatment in women suffering from endometriosis, and compare the effect of two types of conservative surgery on the subsequent response to stimulation. Design: retrospective study with prospective selection of subjects and controls. Subjects: a series of 447 women who underwent IVF. the study group consisted of 119 patients with endometriosis who underwent laparoscopic surgery in an attempt to become pregnant, but failed within a year of surgery. In this group, 77 women had ovarian endometriomas and 42 had peritoneal endometriosis only. The control group consisted of 193 patients with tubal infertility. Measurement: stimulation parameters, fertilization, implantation and pregnancy rates were analyzed in both groups. Results: there was no significant difference among the study subgroups in stimulation parameters or IVF outcome. Conclusions: a similar IVF-ET oucome was observed in patients with endometriosis after destruction or excision of endometriomas compared to women with tubal factor or idiopathic infertility. Thus, endometrioma surgery does not impair IVF outcome if care is taken to respect the normal residual ovarian cortex.
