Abstract
(en) [Decellularization of vascularized digital human grafts: a subunit approach to hand engineering] Digital mutilation, unitary or multiple, has a dramatic impact on hand functionality, leading to a significant decrease in the quality of life. Reconstruction with advanced autologous techniques is limited, and allotransplantation is contraindicated. Our experience in human face graft bioengineering led us to study its application to digit grafts, as a new alternative. Eight human long finger grafts were harvested post mortem, along with their vascular pedicles, and then successfully decellularized by sequential infusion of detergents and polar solvent. Cell clearance was confirmed by standard histology, as well as by a very significant decrease in DNA (95.3 %, p. < 0.0001). The structural preservation of the extracellular matrix (ECM) was observed by microscopy, quantification of bone density level comparable to controls, and measurement of mechanical properties of the digital scaffold taken as a whole, which correlated very closely to those of controls. In vitro, fibroblasts cultured on the matrices demonsrated a preserved viability, with a homogeneous distribution. In vivo, blood reperfusion in a pig recipient showed vascular permeability over 3 hours of observation. We have therefore demonstrated the feasibility of producing complete human digital acellular matrices grafts, retaining their ECM along with an accessible, patent and transplantable vascular tree, opening the road to immuno-compatible grafting and its future clinical applications.
