- (en) [Particularities of use of anti-hyperglycemic treatments in older patients] The care of older diabetic patients has now become a real public health issue due to the increase in the number of patients. In this population, the complications of diabetes are more serious and are intertwined with specific gerontological manifestations. Treatment goals should be individualized based on the patient’s clinical presentation. The new therapeutic classes are particularly interesting because of their effectiveness in terms of cardiovascular and renal protection, but the benefit/ risk balance of these drugs needs to be well assessed according to each person. Insulin therapy is frequently required, either in case of failure of oral antidiabetics or because of the existence of comorbidities, particularly in the event of renal failure. Educating the patient and the patient’s entourage early in the course of the disease is one of the keys to the effectiveness and safety of the treatment. The management of older diabetic patients must avoid both too much laxity in a person who has successfully aged and unreasonable activism in frail subjects because of the hypoglycemic risk.
