RELIABILITY OF TRANSIENT ELASTOGRAPHY FOR PAEDIATRIC LIVER ALLOGRAFT FIBROSIS

Abstract
Transient Elastography (TE) is a non-invasive technique, which estimates the stiffness of a cylindrical volume of liver tissue, 100-fold the size of a standard needle biopsy sample. TE has been validated in adults as predictor of cirrhosis. We aimed to evaluate the reliability of TE for paediatric liver allograft fibrosis. Clinical and biochemical data, TE measurement and histology were reviewed in 99 primary paediatric recipients. Protocol liver biopsies(LB) were assessed by METAVIR (0-4) and by the Liver Allograft Fibrosis Score (LAFSc, 0-9), previously designed and validated specifically for Liver Allograft Fibrosis*. The median liver stiffness measurement was 5.30 KPa (range:2.2-21.3). Of the 99 histologic evaluations, 5 patients did not show fibrosis, scored by METAVIR (median TE values= 4.5 KPa (range 2.2-16.7) and only 1 patient did not show fibrosis by LAFSc (TE value: 2.3 KPa). Significant correlation was found between TE values and severe fibrosis (METAVIR F3 n=21, p= 0.002, and LAFSc (7-9 n=14, p=0.001). But, TE was less accurate for predicting mild and moderate fibrosis (METAVIR F<3 n=72, p=0.04, AUROC 0.65 and LAFSc ≤6 n=84, p=0.01, AUROC 0.74) (AUROC: area under the receiver operating characteristic curve). In Conclusion: Transient Elastography is a promising non‐invasive method for detection of advanced fibrosis/cirrhosis in pediatric liver allograft fibrosis. Prospective studies need to be performed to determine the accuracy of Transient Elastography for the evaluation and follow-up of patients with mild and moderate fibrosis.
