Cognitive-motor interactions in dual-task paradigms in healthy and chronic stroke participants

Abstract
Simultaneous execution of cognitive and sensorimotor tasks is critical in daily life. The clinical usefulness of assessing the performance of a grip-lift task to evaluate motor function in healthy and neurological participants is largely recognised. However, these assessments are always performed in isolation. Here, we use a dual-task paradigm to examine whether dexterous manipulation, a highly habitual and seemingly automatic behaviour, involves high-order cognitive functions, in healthy adults and chronic stroke participants. In dual-tasks, both healthy adults and chronic stroke participants significantly lengthen the initial phase of the movement and strengthen their grip throughout the grip-lift task. The differences were scaled to the needs in cognitive resources. Preload phase and grip forces were significantly increased in both hands of chronic stroke participants, with the strongest increase at the affected hand. However, the cognitive tasks did not appear to exert a stronger effect on motor performance. Cognitive task specifically recruiting working memory interacts with the planning and the on-line control of the fine grip manipulation. Thus dexterous manipulation is not a full automatic behaviour and depends on high-level cognition at some specific moment of its sequence. Assessing motor performance under cognitive load might be a more sensitive way to actually assess motor abilities and their impacts in real life situations.
