Abstract
Graft-versus-host disease (GVHD) is a frequent and severe complication of allogeneic hematopoietic stem cell transplantation caused by uncontrolled donor T cell activity against recipient antigens. Current therapeutic options for acute and chronic GVHD are limited. Clinical approaches under development aim at enhancing immune tolerance by promoting regulatory T cell (Treg) immunosuppression. Tregs suppress other immune cells at least in part by producing active TGF-β1. We recently developed monoclonal antibodies (mAbs) that activate latent TGF-β1 and showed that they exert therapeutic effects in murine model of acute and chronic GVHD. These mAbs increase TGF-β1 activity in lymphoid organs shortly after treatment initiation, appear to improve thymic function and reduce adaptive immune responses after longer term treatment. We suggest that TGF-β1 activating mAbs may serve to treat acute or chronic GVHD in patients.
