Abstract
(en) The prevalence of pituitary adenomas, currently estimated at 1/1200, is likely far higher than previously thought, The clinical work-up should comprise detailed hormonal evaluation, magnetic resonance imaging of the hypothalamic-pituitary region (unless contra-indicated), and most often, an ophthalmologic examination including visual field measurements. The four main types of pituitary adenomas are defined by the hormone secreted in excess: prolactinomas, growth hormone-secreting adenomas (responsible for acromegaly), ACTH-producing adenomas (causing Cushing's disease), and non-functioning pituitary adenomas (NFPAs), the latter representing a heterogeneous group of tumours. The optimal management requires a multidisciplinary approach similar to that used for tumours of the central nervous system, Several efficient therapeutic modalities are currently available, including transsphenoidal neurosurgery, fractionated stereotactic radiotherapy, and various medical treatments, The final strategy should depend on the pituitary adenoma subtype and its resulting complications, Over the past decade, significant progress has been made in the therapeutic approaches for pituitary adenomas, which presently allow for effective tumour and hormonal control in over 90% of the cases
