Abstract
BCG therapy is used to treat non-muscle invasive bladder cancer but its mechanism of action remains evasive. Preclinical studies showed that T cells are necessary for BCG therapy. Here, in four patients treated with BCG, we analyzed T cells infiltrating the tumoral or bladder tissues before and after BCG. We found a tumor-specific T cell response in one tumor, but the frequencies of these T cells did not increase after BCG instillations, neither in the bladder nor in the blood. No tumor-specific CD8+ T cells were found in the three other tumors. Analyses of CD4+ T cells in the bladder tissues of three patients after BCG, but not before, showed a population of cells with an unusual phenotype. Several clonotypes from this population recognized BCG peptides. These results demonstrate the existence of a spontaneous tumor-specific CD8+ T cell response in NMIBC, with no detectable boosting effect of BCG. The latter induces a response of unusual and potentially lytic CD4+ T cells.
