Persistent atrial fibrillation (AF) represents a challenge for the clinician and the evaluation of left atrial (LA) structural remodeling is recommended to optimize its management. In the TRIATLON project (Eudract number: 2019-001813-17), first, we demonstrated the interest of measuring the global peak atrial longitudinal strain and the atrial stiffness index with speckle tracking echography to assess the importance of remodeling and predict AF recurrence. We then studied glucose metabolism through 18F-FDG- positron emission tomography (FDG PET) as a novel tool to assess the extent of structural remodeling. We showed that the use of glucose as energy substrate in the cardiomyocyte was significantly increased during AF. We finally demonstrated that low glucose uptake areas in the LA wall were correlated with the extent of atrial remodeling and predict procedural success after AF ablation procedure. This work confirms the importance of imaging to assess LA structural remodeling and supports the potential role of FDG PET as a new non-invasive tool to assess LA structural remodeling (and atrial cardiomyopathy) and improve the management of persistent AF patients.
