Chronic rhinosinusitis (CRS) is a heterogeneous inflammatory disease of the upper airways. This thesis explores the role of secretory IgA (s-IgA) in CRS pathophysiology using an integrated translational approach. Combining human sinonasal tissue analyses with innovative murine models of eosinophilic and neutrophilic CRS, the study reveals impaired epithelial pIgR/IgA expression and s-IgA in secretions in patients, as well as divergent effects of s-IgA deficiency on inflammation across endotypes. Moreover, we found that s-IgA seem to bind nasal microbiome members differently among patients and controls, suggesting differences in immune response towards commensals. These findings suggest an important and currently neglected role of s-IgA in driving CRS.
