Abstract
In chronic severe aortic or mitral regurgitation, patients progressively develop cardiac dysfunction, ultimately resulting in increased morbidity and impaired survival. Surgical management of these valvular conditions intends to restore normal life expectancy and quality of life. Nonetheless we still miss a marker reliably reflecting myocardial damage, and especially the tipping point beyond which irreversible myocardial injury occurs. Consequently, defining the ideal timing to refer patients for surgery remains a challenge. In this work, we proceeded from postoperative evolution of patients to pinpoint subclinical myocardial damage existing at the time of surgery, and from there aimed to identify preoperative clues of such dysfunction. We determined that left ventricular volumes were highly predictive of postoperative course. By contrast, our strain and stress analyses provided little added value.
