The hemostatic system in patients with cirrhosis is complex with alterations in pro-hemostatic and anti-hemostatic drivers leading to so-called rebalanced hemostasis. Literature on the hemostatic system in pediatric cirrhosis is limited, particularly regarding primary hemostasis. To address this gap, we conducted a proof-of-principle study to inform future, clinically focused research. We investigated the hemostatic system—including primary hemostasis, the coagulation cascade, and fibrinolysis — in a cohort of pediatric patients with cirrhosis, just before and 3 months after living donor liver transplantation. Primary hemostasis was assessed using both flow cytometry and a dynamic microfluidic assay. Overall, pediatric patients with cirrhosis displayed a relatively rebalanced hemostatic system before liver transplantation, with primary hemostasis varying according to platelet count, von Willebrand factor levels, and disease severity.
