Abstract
Recently, some clinical studies have investigated the feasibility and effectiveness of coupling nebulization of drugs with noninvasive positive pressure ventilation (NIPPV) in patients with respiratory diseases. The objective of this thesis was to evaluate the effect of an experimental inspiratory synchronized nebulization on aerosol delivery during NIPPV using single-limb circuit bilevel ventilators. In vitro measurements were realized by residual gravimetric and laser diffraction methods. Pulmonary delivery was investigated by pharmacokinetic study in healthy volunteers. During NIPPV, in vitro comparison of 3 types of constant-output nebulizers showed that the inhaled dose was greater with the vibrating mesh nebulizer than with the jet and ultrasonic nebulizers, regardless of their location relative to the passive exhalation port. The use of our experimental inspiratory synchronized vibrating mesh nebulization improved the respirable dose compared to continuous vibrating mesh nebulization. The pharmacokinetic study in healthy volunteers undergoing NIPPV confirmed these findings. The studies carried out in this thesis demonstrated that inspiratory synchronized nebulization significantly increased pulmonary drug delivery, thus we strongly hope that such systems will be developed in the near future. However, clinical trials in patients with pulmonary diseases are required to determine if these results are clinically relevant.
