Diagnostic et prise en charge de la variabilité de la réponse au clopidogrel dans l'artériopathie oblitérante des membres inférieurs: Illustrations cliniques et revue de la littérature
(en) [Diagnosis and management of variable clopidogrel response in obliterating arteriopathy of the lower limbs: clinical illustrations and literature review] We report the case of two patients with diabetes mellitus who attended our hospital consultation for chronic peripheral artery disease (stent thrombosis) in spite of antiplatelet drug treatment (clopidogrel 75mg, aspirin 8 mg). A blood aggregometry with ADP was performed revealing high residual platelet reactivity. A genetic polymorphisms in the hepatic enzymes (CYP2C19 ; PON-1) involved in clopidogrel metabolism was identified. After adjusting the clopidogrel dose, both a biological and clinical response was observed.
