- Abstract
- Efficacy of antiviral therapy in hepatitis C related cirrhosis has been investigated in a Limited number of therapeutic trials. Nearly all these trials have been conducted in compensated cirrhosis and, unfortunately, their end-points were not identical. Conflicting data have been obtained with regard to a potential advantage for interferon therapy in term of normalization in Liver function tests, disappearance of serum HCV RNA and/or the risk of hepatocarcinoma development. Zn deconpensated cirrhosis, the use of antiviral therapy is not recommended. In conclusion, antiviral therapy in cirrhotic HCV patients should only be conducted on the basis of prospective, randomized clinical trials in which efficacy in term of virologic effect, incidence of complications and survival must be carefully investigated taking also into account the incidence of side effects and the cost of antiviral drugs.
