Squamous cell carcinoma of the head and neck (SCCHN) is a heterogeneous disease with diverse primary locations, etiologies, treatment approaches, and patterns of relapse. At the recurrent and/or metastatic stage, platinum-based chemotherapy, PD-1 inhibitors, and cetuximab can improve survival; however, the prognosis remains poor. To enhance treatment outcomes and patient survival, a deep understanding of tumor biology and its microenvironment is essential. Currently, our understanding of the molecular landscape of head and neck cancer is largely derived from studies of primary tumors, and the mechanisms of resistance to PD-1 inhibitors in SCCHN remain poorly understood. In this thesis, we aim to provide a comprehensive biological characterization of recurrent and/or metastatic SCCHN and to identify potential mechanisms of resistance to PD-1 inhibitors, offering critical new insights that may improve patient treatments and outcomes.
