Abstract
Objective: 1) To describe a new instrument (GyneLase®), a new approach (ELITT™ Endometrial Laser Intra-uterine Thermal Therapy) in the treatment of menorrhagia. 2) To evaluate the efficacy of this new technique in the management of dysfunctional uterine bleeding (DUB). Design: Prospective study. One hundred patients followed for one year. Patients: One hundred premenopausal women suffering from DUB. Measurement: Amenorrhea rate after one year. Results: The amenorrhea rate after one year of follow-up is 71% and the amenorrhea/ severe hypomenorrhea rate is more than 90%, much higher than the rates observed in the literature after other procedures like electrosurgery or intrauterine thermal balloon. The ELITT™ procedure offers an inherently safe and simple alternative, providing controlled and effective treatment of the entire endometrium. In contrast to traditional endometrial ablation with Nd:YAG laser, the ELITT™ procedure does not require either intensive training nor hysteroscopic control, and it is far less risky as the power used per unit area is 1.000 times lower. Conclusion: The ELITT™ procedure is a new non-hysteroscopic technique for endometrial ablation, the technique is very safe and offers the highest amenorrhea rate so far described in the literature.
