Deprescribing benzodiazepine receptor agonists in hospitalised older adults : from the description of actual practices to the development of a targeted intervention
Benzodiazepine receptor agonists (BZRAs) are considered inappropriate to manage insomnia in older adults. The hospital setting provides an opportunity for BZRA deprescribing (supervised dose reduction or cessation) for several reasons. Firstly, BZRA use remains high in hospitalised older adults living with polypharmacy and multimorbidity. Secondly, BZRA deprescribing is moderately implemented: there is room for improvement, especially regarding specific subpopulations such as (central nervous system-acting) polypharmacy. A BZRA-specific intervention was developed for physicians, considering context and specific barriers. Promoting proactive deprescribing, shared decision making and care transition are the corner stones of this intervention. The intervention was developed following robust frameworks and behavioural theories to enhance its implementation, dissemination and replicability.
